1. To return a maiden name, address to body of the REGISTRY OFFICE where marriage was registered, and write the statement in which specify: - surname, name, middle name, birth date and birth place, nationality, nationality, residence, marital status; - a surname, a name, a middle name, date of birth of all children who did not reach full age age; - data of marriage certficates or on a divorce which were issued earlier; - data of birth certificates of children; - a surname which you want to take; - reason of change of a surname.
2. That your statement was taken cognizance, provide the following documents: - the birth certificate; - certificate on divorce or death certificate of the spouse; - birth certificates of the children who did not reach full age age.
3. The staff of body the REGISTRY OFFICE has to consider your application within 30 calendar days. In the presence of good reasons this term can be increased up to 2 months. Besides, if you submitted the application for change of a surname not to that body of the REGISTRY OFFICE which to you granted the certificate on marriage, and in another, then this term, according to the legislation, can be increased up to 3 months. It is connected with the fact that employees have to make at first request in that body of the REGISTRY OFFICE in which you registered marriage, and only after receiving confirmation from them they can begin consideration of your statement.
4. After the specified term to you the certificate on change of a surname will be granted. In birth certificates of minor children your surname will also be changed.
5. After obtaining the certificate on change of a surname address in a passport office in a place of registration for replacement of the all-civil passport. On its basis change also all other documents: INN, pension insurance certificate, health policy, driver's license, international passport and so on.
